Custom made chest implant for a patient with Poland’s syndrome.
A 35-year-old male with left chest Poland’s Syndrome shown here 3 months post-procedure. Procedure: In-office creation of moulage of the chest wall defect followed by insertion of a custom made, sterile, semi-solid silicone implant via an axillary approach using an endoscopic technique. Results: The patient’s chest shows a good proportion and symmetry. He is very athletic and has resumed all activities.

26 year old male with right side affected Poland Syndrome. He had partial absence of his right pectoralis major muscle as well as a complete absence of his nipple, medically called athelia. Note the well-developed clavicular portion of his pectoralis major muscle in compensation for the absent sternal attachments of the pectoral muscle. He underwent a custom made semi-solid silicone implant placement via an axillary incision. He then underwent creation of the nipple in the office with local anesthesia, followed by nipple and areolar tattooing to match his opposite chest.

23 year old female with left side affected Poland Syndrome with amazia, or complete absence of breast but present nipple areolar complex. She had partial absence of her pectoralis major muscle where the central and inferior pectoral muscle was not developed; but the clavicular fibers were intact. Note also the smaller and upper and lateral location of the nipple areolar complex. She underwent staged reconstruction with placement of a tissue expander for the first stage. This allowed slow expansion of the soft tissues of the left chest until a desired size was reached. She desired a slightly fully right breast as well. For the second stage in her Poland Syndrome reconstruction, she underwent an exchange of the tissue expander to a permanent breast implant by replacing it with a 385cc High Profile silicone gel implant on the left and a 190cc Low Profile silicone implant on the right. Fat grafting (135cc) to the left upper breast pole and lateral breast region was also performed at the time of the tissue expander exchange for better symmetry. She wears a 34C brasserie and is very pleased with the outcome.

Note presence of his clavicular attachment but absent sternal attachments of the fan shaped pectoral muscle. He did not have other associated congenital abnormalities. He underwent a custom-made semi-solid silicone implant placement via an axillary approach. Note good symmetry and contour with his opposite chest.

He had partial absence of the inferior and central fibers of his right pectoralis major muscle with intact clavicular segments. 35 year old right side affected male with Poland Syndrome with eccentric and inverted small nipple areolar complex. He underwent trans axillary placement of a custom made semi solid silicone implant for improved chest symmetry. Six months after surgery he returned and had excision of the diminutive, inverted nipple, and creation of a new nipple in the office with local anesthesia. The after photos were taken by our patient. He will return for tattooing to complete the reconstruction.
