Regaining a Youthful Facial Contour

The face is the focal point of our appearance. Over time, it can start to show signs of aging in the form of sagging and volume loss. Factors such as genetics, sun exposure, gravity, and lifestyle choices can all contribute to the development of wrinkles, skin laxity, and hollowing. While these changes are a natural part of the aging process, they can have a significant impact on our self-confidence and overall sense of well-being.

Facial aging is a multidimensional process involving three key factors; skin, soft tissue, and bone. (1) Fortunately, advancements in cosmetic surgery have provided us with a range of options to address these concerns and restore a more youthful and rejuvenated appearance

A facelift is a surgical procedure that aims to reverse the effects of aging by tightening the facial muscles, repositioning the descended fat and removing excess fat and skin to create better definition and a smoother and more youthful contour. The procedures involved result in a comprehensive approach to facial rejuvenation, addressing multiple areas of concern such as deep and fine wrinkles, jowls, sagging skin, and loss of facial volume.

Combining a Facelift with a Neck Lift

Besides facial descent and volume loss, many people are concerned with sagging and loose skin around the neck. In most instances, we use a combination of tightening of the neck muscles (platysmal plication or a platysmaplasty) and removal of excess neck skin as well as liposuction, which removes unwanted fatty deposits. This sculpts the neck and gives it excellent definition and contour and a younger appearance. All incisions are carefully designed to be well-hidden and imperceptible to the naked eye.

Can Non-Surgical Procedures Restore My Facial Appearance?

It is important to note that while non-surgical facelift options may offer temporary improvements, they cannot provide the same level of comprehensive and long-lasting results as a surgical facelift. Surgical intervention allows for more significant changes in facial contour and can address underlying muscle laxity and excess skin more effectively.

Benefits of a Facelift

When it comes to achieving a more youthful and rejuvenated appearance, a facelift performed by Dr. Salibian is an excellent option. One of the primary advantages of a facelift is the youthful look it provides and natural appearance. It can minimize the appearance of jowls, deep smile lines around the mouth, and a double chin, which are common signs of aging. Dr. Salibian’s expertise and meticulous approach ensure natural-looking results that are tailored to your unique facial anatomy and aesthetic goals. By addressing sagging skin, wrinkles, and other signs of aging, a facelift can help you achieve a smoother, youthful and more vibrant facial appearance.

In addition to its cosmetic benefits, a facelift also offers functional improvements. The procedure involves tightening facial muscles, resulting in more toned and well-defined facial features. This not only improves your facial contours, silhouette, and proportions but also contributes to a firmer and more youthful skin texture.

The long-lasting results of a facelift are another significant advantage. With proper maintenance and care, the effects of the procedure can endure for ten years or more. This means you can enjoy your rejuvenated and refreshed facial appearance for many years to come in good health and vitality.

What is the SMAS?

The SMAS, also known as the Superficial Musculo-Aponeurotic System, is a vital anatomical structure in the face that plays a pivotal role in facelift procedures. It is intricately connected to facial anatomy, consisting of a network of muscles and fibrous tissues that extend throughout the face, providing support and structure.

One of the techniques involving the SMAS is SMAS plication, which involves tightening and repositioning the underlying tissues to achieve a more youthful appearance. Facelift procedures that include manipulation of the SMAS are proven to have longer-lasting results than skin-only facelifts. (2)

Facelift Procedures

Traditional Facelift

During a traditional facelift procedure, incisions are strategically made along the hairline and behind the ears, allowing the surgeon to access and manipulate the underlying facial tissues. The next stage involves repositioning the underlying facial tissues to achieve a more youthful contour. This includes lifting and tightening the muscles and connective tissues beneath the skin. In doing so, the surgeon can provide a firmer foundation for the skin to be redraped over.

After the tissue repositioning process, the skin is redraped with the appropriate tension over the newly tightened facial structure, and the excess skin is removed. This step helps to eliminate any visible sagging or wrinkling, resulting in a smoother and firmer overall appearance.

To secure the newly positioned tissues and close the incisions, the surgeon utilizes discrete sutures. These sutures not only promote optimal healing but also help to minimize scarring.

Following the traditional facelift procedure, patients will undergo a recovery process that involves post-operative care, monitoring, and scheduled follow-up visits. This ensures proper healing and allows us to monitor the progress and results of the surgery.

Endoscopic Mid-Facelift

The endoscopic mid-facelift raises the cheeks and mid-face for a more youthful look. In youth, we have a shorter distance between the lower eyelash line and the mid-cheek. With time and gravity, the mid-cheek descends. A mid-face vertical lift procedure (often referred to as a deep plane facelift) repositions the cheek and mid-face. This improves tissue descent and lower eyelid hollows.

If lower eyelid skin laxity needs to be addressed simultaneously, a lower eyelash incision (right below the lower eyelash line) is made to correct both the position of the mid-face as well as the excess lower eyelid. This incision heals without visible scars.

In patients where the lower eyelid does not need to be surgically addressed, an endoscopic approach alone is used to reposition the cheek and mid-face. The incisions are hidden within the temporal hairline and a counter-incision is made inside the cheek to assist in easy repositioning of the mid-face.

The unique advantage of this approach is that the incisions are tiny and hidden in the hair line and there is minimal numbness or bruising, if at all. In general, the recovery is faster in an endoscopic mid-facelift than in the traditional face and neck lift as there is no excess skin removal. However, the swelling at the mid-cheeks persists somewhat longer than in the traditional SMAS – Facelift approach.

Male Facelift

A male facelift focuses on enhancing masculine features and creating a strong and well-defined jawline. It is a specialized form of facial rejuvenation that is tailored to the unique needs and aesthetic goals of men. As men age, they may experience loss of facial volume, hair, sagging skin, deep wrinkles, and a poorly-defined jawline. These changes can often make men appear tired, older, or less vibrant than their actual selves.

Dr. Salibian understands the unique needs and goals of his male patients when it comes to facial aesthetics. With a male facelift, patients can achieve enhanced and rejuvenated chiseled features. With his expertise and artistic approach, Dr. Salibian offers a comprehensive range of male aesthetic procedures to achieve a natural-looking and youthful appearance.

Fat Grafting to the Face

Fat grafting to the face, also known as autologous fat grafting or fat transfer, is a useful technique that may be employed during facelift procedures to address volume loss. It offers a natural solution by utilizing your body’s own fat to restore volume and rejuvenate your facial features. By integrating fat grafting into a facelift procedure, Dr. Salibian can address multiple concerns simultaneously. Not only does a facelift lift and tighten sagging skin, but the addition of fat grafting enhances facial symmetry, restores lost volume, and improves the overall balance of your features. The result is a more youthful, vibrant appearance that looks entirely natural.

Corresponding & Complementary Procedures

A procedure commonly performed with a face and neck lift is an eyebrow or forehead lift. This procedure elevates the heaviness and sagging as well as the deep furrows of the brow and forehead, eyelid surgery to rejuvenate aging eyes, and procedures that address facial skin tone, texture, and pore size, such as laser facials and laser treatments to eliminate facial blood vessels and brown spots as sun or age spots.

In many instances, the use of Sculptra or other means of volume restoration such as fat transfer will further extend the longevity of a facelift even when performed several years after the initial facelift. Fat transfer is often used in conjunction with mid-facelift both in endoscopic or traditional techniques. Fat grafting improves the volume loss caused by aging where a repositioning of the cheeks and tightening of the neck muscles and skin alone will not address.

Who is a good candidate for a facelift?

Ideal candidates for a facelift are individuals who are generally healthy, have realistic expectations, and are bothered by visible signs of facial aging. While there is no specific age requirement, most patients seeking a facelift are between the ages of 45 and 75. However, individual factors such as skin quality, bone structure, and overall health will be taken into consideration during the consultation process.

What is the recovery process like after a facelift?

After a facelift, patients can expect some swelling, bruising, and mild discomfort. Proper post-operative care, including rest, elevation, and following the surgeon’s instructions, will help facilitate a smooth recovery. Most patients are able to return to work and social activities within 10 to 14 days, although strenuous physical activities should be avoided for several weeks.

Can a Facelift be combined with other procedures?

Yes, a facelift can often be combined with other procedures to enhance overall facial rejuvenation. Commonly paired treatments include neck lifts, brow lifts, eyelid surgery, and fat grafting. Combining procedures allows for a more comprehensive transformation, addressing multiple areas of concern and achieving harmonious results.

How long do the results of a facelift last?

While a facelift cannot stop the natural aging process, it can turn back the clock by several years. The longevity of the results will vary depending on individual factors such as genetics, lifestyle choices, and skincare routine. However, most patients can enjoy their rejuvenated appearance for approximately 10 to 15 years before considering additional touch-ups or maintenance treatments.

Do patients go home after a facelift procedure?

After the procedure, patients will be admitted to a cosmetic recovery center for monitoring before being released to go home.
