48 year old patient with locally advanced left breast cancer who underwent left skin sparing mastectomy with placement of a Tissue Expander in the subpectoral position with Acellular Dermal Matrix for support as Stage I Tissue Expander – Permanent Breast Implant Breast Reconstruction. She then underwent a series of post mastectomy left breast chest wall and axillary radiation treatments. Six weeks post completion of radiation treatments, she had biweekly expansions until her desired breast size was reached. During her Stage II Breast Reconstruction, the Tissue Expander was replaced with a 650cc High Profile Silicone Gel breast implant. Internal breast capsular work was performed to further improve the natural appearance of the reconstructed breast and same time right breast reduction / lift was performed via a Vertical Incision only medial pedicle technique, otherwise known as a “lollipop” incision breast lift / reduction. Note the discoloration on her left chest wall from the radiation treatments that have faded to an extent. She has good symmetry, shape, and contour between her reconstructed and native breasts. She used to be a 38 DD cup before her surgeries and is satisfied with her desired 38 full D cup size breasts.

50 year old patient with history of right breast cancer who underwent a skin sparing mastectomy with same time placement of Tissue Expander in the subpectoral position with Acellular Dermal Matrix for support. Post tissue expansion, she underwent exchange of the tissue expander to a permanent breast implant using a High Profile 700cc silicone gel breast implant with same time symmetrizing left breast lift. Photos are relatively recent post-op but show good symmetry with matching projection and contour.

59 year old patient with locally advanced right breast cancer for which she underwent neoadjuvant chemotherapy followed by modified radical mastectomy with same time single stage right breast reconstruction with latissimus myocutaneous flap without implant. She had adequate amount of upper back adipose (fat) tissue that did not require additional use of a breast implant for projection, and she was very content with the result. She may consider having a left breast symmetrizing lift and creation of a new right-side nipple and tattooing in the future.

Tissue expanders were placed in the subpectoral position with Acellular Dermal Matrix for support of the lower poles. During her second stage, the tissue expanders were replaced with 650cc High Profile silicone gel breast implants and had conservative amount of fat grafting to the upper poles of her breasts for better aesthetics. She was satisfied with the results and did not care to have nipples and areolae created or tattooing.

63 year old patient with bilateral invasive lobular breast cancer, Stage 1, who elected to have bilateral nipple-sparing mastectomy with bilateral breast reconstruction as a Single Stage operation. She underwent an autologous breast reconstruction with a bilateral TRAM flap where each half of her lower abdominal tissues were used as a donor site to replace and reconstruct each one of her breasts. The mastectomy skin incisions were designed as a Wise pattern like a breast reduction and her original nipple and areolar complex was preserved and replaced as a full thickness skin graft over the reconstructed breasts. Her breast size was reduced from a 36DD to a 36D cup size post reconstruction.  The abdominal TRAM flap donor site was repaired using both in-lay and an on-lay polypropylene mesh to preserve her abdominal muscle integrity and support her core rotational movement strength. The post operative photo here was only two months post-surgery showing some residual swelling at her lower abdomen that will dissipate within the next three more months. Her breast reconstruction appears natural, like a mommy makeover patient that had a breast lift / reduction and a tummy tuck.

39 year old patient with right sided early stage breast cancer who underwent oncoplastic breast reduction at the same time as her right breast lumpectomy (quadrantectomy). Her left breast reduction was done via a Wise skin pattern inferior breast pedicle; whereas her right breast reduction was done similarly via a Wise pattern skin paddle but using a superior lateral breast pedicle, as the lumpectomy was from the medial inferior breast quadrant.
