30 year old patient who had had breast augmentation with 510cc silicone gel implants in the subglandular position via a supra-areolar incision ten years prior with another surgeon. She had developed severe, Grade IV capsular contracture with suspected implant rupture. She underwent a total capsulectomy due to the thickened breast capsules (worse at her left than at her right) and removal of her ruptured silicone implants and same time placement of new 421cc Moderate Profile silicone gel implants in the dual plane / sub pectoral position with a peri areolar vertical and short scar horizontal breast lift to achieve the final 36 full D cup soft and natural appearing breasts that she desired.

70 year old patient who had had breast augmentation greater than 30 years ago with progressive hardening and capsular contracture and pain. She underwent a total capsulectomy “en bloc capsulectomy” as her breast capsules were thickened with calcified white debris within the capsular shell itself. She had intracapsular rupture of both of her silicone 275cc implants. To address breast sagging, she also underwent a same time vertical incision only or a “lollipop” breast lift with moderate amount of fat grafting along her medial cleavage regions. Her chest feels soft and comfortable, and she is thrilled with the results especially due to return of sensation, both sensory and erogenous to her nipples that she did not remember having for decades.

Therefore she underwent a bilateral breast revision with removal and replacement of the Extra High Profile Highly Cohesive silicone gel implants to a Moderate Profile 485cc natural feel silicone implants (not as highly cohesive and heavy) with internal lift of the breast pocket performed via a periareolar incision with a periareolar breast lift.
