48 year old patient with locally advanced left breast cancer who underwent left skin sparing mastectomy with placement of a Tissue Expander in the subpectoral position with Acellular Dermal Matrix for support as Stage I Tissue Expander – Permanent Breast Implant Breast Reconstruction. She then underwent a series of post mastectomy left breast chest wall and axillary radiation treatments. Six weeks post completion of radiation treatments, she had biweekly expansions until her desired breast size was reached. During her Stage II Breast Reconstruction, the Tissue Expander was replaced with a 650cc High Profile Silicone Gel breast implant. Internal breast capsular work was performed to further improve the natural appearance of the reconstructed breast and same time right breast reduction / lift was performed via a Vertical Incision only medial pedicle technique, otherwise known as a “lollipop” incision breast lift / reduction. Note the discoloration on her left chest wall from the radiation treatments that have faded to an extent. She has good symmetry, shape, and contour between her reconstructed and native breasts. She used to be a 38 DD cup before her surgeries and is satisfied with her desired 38 full D cup size breasts.

50 year old patient with history of right breast cancer who underwent a skin sparing mastectomy with same time placement of Tissue Expander in the subpectoral position with Acellular Dermal Matrix for support. Post tissue expansion, she underwent exchange of the tissue expander to a permanent breast implant using a High Profile 700cc silicone gel breast implant with same time symmetrizing left breast lift. Photos are relatively recent post-op but show good symmetry with matching projection and contour.

52 year old patient with right sided breast cancer who elected to undergo a bilateral skin sparing mastectomy with same time Staged Tissue Expander to Permanent Breast Implant reconstruction. Tissue expanders were placed in the subpectoral position with Acellular Dermal Matrix for support of the lower poles. During her second stage, the tissue expanders were replaced with 650cc High Profile silicone gel breast implants and had conservative amount of fat grafting to the upper poles of her breasts for better aesthetics. She was satisfied with the results and did not care to have nipples and areolae created or tattooing.

47 yo 5′ 7″ underwent left mastectomy and same time tissue expander breast reconstruction, full muscle coverage. Second stage – replacement of expander with anatomic shaped 700cc saline textured implant and contralateral (right) breast symmetrizing reduction. Third stage – creation of left breast nipple and fourth stage – tattooing of nipple and areolar complex. Note symmetry in size, shape, contour and overall proportion to body habitus.

Bilateral tissue expander breast reconstruction / creation of nipple and areolar complexes.
41 yo 5′ 5″ had right breast cancer requiring a mastectomy as well as radiation to the right chest wall and axillae. She elected to have a bilateral mastectomy (prophylactic on her left) and same time insertion of tissue expander breast reconstruction – full muscle coverage. On second stage she underwent replacement with silicone gel high profile implants; right (radiated side) with a 500cc HP silicone gel impant and her left with a 400 cc HP silicone gel implant. We also created her nipple and areolar complexes at the same time as the implant replacement of the tissue expanders using inner thigh skin for the areolas as full thickness skin grafts. The darker right breast skin color and the oblique scar on the right breast is from the radiation treatment and initial biopsy of her right breast cancer. Otherwise the breasts feel soft, including her right, appear symmetrical and in proportion to her overall body shape and size at 36 “C” cup.

31 year old patient who had undergone a right modified radical mastectomy for locally aggressive breast cancer and post mastectomy radiation two years prior to her initial presentation for breast reconstruction. She underwent staged breast reconstruction where a tissue expander was inserted in a retro-pectoral position with use of acellular dermal matrix for support. She underwent slow progressive tissue expansion given the history of radiation to the right chest wall. After eight months of expansion, she underwent the second stage breast reconstruction where the tissue expander was removed, and the internal breast pocket was adjusted to accommodate a 650cc high profile silicone gel breast implant. She also underwent a same time symmetrizing left breast vertical incision only or a lollipop breast lift. Of note, she had an extra numerary right nipple along the lower half of her right breast that she did not want excised!
