31 year old patient who had undergone a right modified radical mastectomy for locally aggressive breast cancer and post mastectomy radiation two years prior to her initial presentation for breast reconstruction. She underwent staged breast reconstruction where a tissue expander was inserted in a retro-pectoral position with use of acellular dermal matrix for support. She underwent slow progressive tissue expansion given the history of radiation to the right chest wall. After eight months of expansion, she underwent the second stage breast reconstruction where the tissue expander was removed, and the internal breast pocket was adjusted to accommodate a 650cc high profile silicone gel breast implant. She also underwent a same time symmetrizing left breast vertical incision only or a lollipop breast lift. Of note, she had an extra numerary right nipple along the lower half of her right breast that she did not want excised!
