What is the SMAS?

The SMAS, also known as the Superficial Musculo-Aponeurotic System, is a vital anatomical structure in the face that plays a pivotal role in facelift procedures. It is intricately connected to facial anatomy, consisting of a network of muscles and fibrous tissues that extend throughout the face, providing support and structure.

One of the techniques involving the SMAS is SMAS plication, which involves tightening and repositioning the underlying tissues to achieve a more youthful appearance. Facelift procedures that include manipulation of the SMAS are proven to have longer-lasting results than skin-only facelifts. 

Facelift Procedures

Traditional Facelift

During a traditional facelift procedure, incisions are strategically made along the hairline and behind the ears, allowing the surgeon to access and manipulate the underlying facial tissues. The next stage involves repositioning the underlying facial tissues to achieve a more youthful contour. This includes lifting and tightening the muscles and connective tissues beneath the skin. In doing so, the surgeon can provide a firmer foundation for the skin to be redraped over.

After the tissue repositioning process, the skin is redraped with the appropriate tension over the newly tightened facial structure, and the excess skin is removed. This step helps to eliminate any visible sagging or wrinkling, resulting in a smoother and firmer overall appearance.

To secure the newly positioned tissues and close the incisions, the surgeon utilizes discrete sutures. These sutures not only promote optimal healing but also help to minimize scarring.

Following the traditional facelift procedure, patients will undergo a recovery process that involves post-operative care, monitoring, and scheduled follow-up visits. This ensures proper healing and allows us to monitor the progress and results of the surgery.

Endoscopic Mid-Facelift

The endoscopic mid-facelift raises the cheeks and mid-face for a more youthful look. In youth, we have a shorter distance between the lower eyelash line and the mid-cheek. With time and gravity, the mid-cheek descends. A mid-face vertical lift procedure (often referred to as a deep plane facelift) repositions the cheek and mid-face. This improves tissue descent and lower eyelid hollows.

If lower eyelid skin laxity needs to be addressed simultaneously, a lower eyelash incision (right below the lower eyelash line) is made to correct both the position of the mid-face as well as the excess lower eyelid. This incision heals without visible scars.

In patients where the lower eyelid does not need to be surgically addressed, an endoscopic approach alone is used to reposition the cheek and mid-face. The incisions are hidden within the temporal hairline and a counter-incision is made inside the cheek to assist in easy repositioning of the mid-face.

The unique advantage of this approach is that the incisions are tiny and hidden in the hair line and there is minimal numbness or bruising, if at all. In general, the recovery is faster in an endoscopic mid-facelift than in the traditional face and neck lift as there is no excess skin removal. However, the swelling at the mid-cheeks persists somewhat longer than in the traditional SMAS – Facelift approach.

This procedure elevates the heaviness and sagging as well as the deep furrows of the brow and forehead, eyelid surgery to rejuvenate aging eyes, and procedures that address facial skin tone, texture, and pore size, such as laser facials and laser treatments to eliminate facial blood vessels and brown spots as sun or age spots.

In many instances, the use of Sculptra or other means of volume restoration such as fat transfer will further extend the longevity of a facelift even when performed several years after the initial facelift. Fat transfer is often used in conjunction with mid-facelift both in endoscopic or traditional techniques. Fat grafting improves the volume loss caused by aging where a repositioning of the cheeks and tightening of the neck muscles and skin alone will not address.
