20 year old patient who had seen me for bilateral mastectomy as she is a BRCA – II gene mutation carrier (a high risk of developing breast and ovarian cancer in her lifetime). Given her large, heavy and pendulous breasts and without a current personal history of breast cancer we decided that she would first undergo breast reduction surgery to a size that she would be both functionally and aesthetically pleased with and later have a prophylactic nipple sparing mastectomy with bilateral breast reconstruction. This way at the time of the mastectomy she will not have any new incisions on her breasts and both the mastectomy and reconstruction can be accomplished using her inframammary fold scar (horizontal scar at her breast fold), preserving her existing nipple and areolar skin and overall shape and look of her breasts. With this approach she would enjoy having her native natural breasts, currently downsized from a pre op size of 38DDD to a 38D cup, until she is ready to have them removed and reconstructed as a preventative measure to avoid having breast cancer in the future.
