53 year old patient with a history of right breast cancer treated 10 years prior via lumpectomy and radiation – resulting in a contracted right lower pole – presented with recurrence at the same breast. She underwent a right skin-sparing mastectomy with immediate tissue expander placement, alongside a prophylactic left nipple-sparing mastectomy with direct-to-implant reconstruction. The left side procedures utilized an inframammary fold incision. Postoperative photographs show results following the second stage exchange of the right tissue expander to a permanent 450cc high-profile silicone gel implant, paired with a 425cc left breast implant, achieving the patient’s desired 36C cup size.
