49 year oldwith locally advanced right breast cancer who had elected to undergo bilateral mastectomyand required post mastectomy radiation to right chest wall. She underwent delayed bilateral latissimusflap with staged tissue expander reconstruction. Second stageshe underwent fat grafting to the rightand left breast upper poles during exchange of the tissue expanders to permanent breast silicone gelimplants (450cc high profile). She did not elect to undergo nipple reconstruction.
