51 yo 5′ 11″ with history of previous mastectomy had chest wall recurrence which required chest wall resection to achieve margin free status prior to irradiation. Optimal chest wall resection alone to obliterate local recurrence would not have been possible by resection alone had a large latissimus flap not been used to cover the resultant defect. In using the latissimus flap as a “life boat” for chest wall salvage post massive resection allowed safe and optimum delivery of post operative radiation as an integral component of the chest wall recurrent cancer treatment regimen.
