A 68-year-old Caucasian female was diagnosed with early-stage breast cancer on the right side. Due to her large, pendulous breasts and significant sagging, the patient underwent a right breast lumpectomy performed by the breast oncological surgeon and simultaneous bilateral breast reduction. The breast reduction was performed using a Wise pattern skin closure, and her nipple and areolar complexes (NAC) were preserved as split-thickness skin grafts placed over the reduced and contoured breast mounds. A traditional breast lift, which preserves the blood supply to the NAC, was not suitable in this case because of the extensive lift required and the need to maintain symmetry and a good surgical outcome alongside the lumpectomy and reduction. While it is uncommon to use skin grafts for the NAC in breast reduction procedures, this approach was considered optimal for her unique presentation.
